Shokunin Performance Rehab provides cash pay physical therapy services and is an out-of-network provider. Please contact your insurance company regarding your out-of-network benefits because you could get reimbursement by submitting a superbill.
We are currently not accepting Medicare and other federal plans, and are unable to see Medicare qualified clients at this time.

Direct access physical therapy refers to the ability of patients to seek direct treatment from a licensed physical therapist without the need for a physician's referral or prescription. Oregon is a direct access state for physical therapy services. As a result of legislative changes effective January 1, 2014, a physical therapist is no longer required to limit treatment of a self-referred patient to 60 days before having to refer the patient to a provider identified in ORS 688.132(1).
